speech therapy

Requesting a Prescription for Speech Therapy

1. Call your child’s primary care physician and state that you have concerns and would like a referral for speech-language therapy.

2. Ask the doctor’s office to fax the prescription to 404-795-0840.

3. The prescription will need to have a Diagnostic Code. Codes typically used for Speech-Language Therapy include

  • F80.0 Articulation/Phonological Disorder

  • R62.0 Late Talker

  • R48.2 Apraxia

  • R47.1 Dysarthria

  • M26.59 Dentofacial Functional Abnormalities, Malocclusion due to abnormal swallowing

  • Q38.1 Tongue Tie

  • F81.81 Specific Disorder of written expression

  • F80.1 Expressive Language Disorder

  • F80.2 Mixed Receptive-Expressive Language Disorder

  • H93.25 Central Auditory Processing Disorder

  • F80.4 Speech and Language Developmental Delay due to hearing loss
